As quoted in the quest itself you need to kill world boss and enemies in pandemonium

and killing in pandemonium is really hard to get

The black scale is bugged. Nobody has received it yet, including those who have got the killing blow on the boss. I personally have had the final blow about 6 times and no black scale.
